CommonHealth Partners will award mini-grants of up to $1000 per applicant. Applicants must submit innovative projects that:
- Connect older adults living in the Historic Richmond Region who are at risk of isolation with resources that help mind, body and spirit THRIVE. - Promote safe, sterile interaction - Demonstrate sustainability - Are strength and asset-focused - Include team members who are under-employed/resourced - Include an itemized budget
A final report will be required that demonstrates number of older adults served, type of service and efficacy/results. Videos that may be shared are appreciated throughout these projects.
